Your visit

Your appointment will begin with a consultation explaining your diagnosis and treatment options. Surgery can often be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your appointment:

  • Your referral slip and any X-rays from your doctor

  • A list of current medications

  • If you have medical and/or dental insurance, please bring your insurance cards. Accurate information will help us process any claims.

  • Medical records if you have a complex medical history

For your convenience, you may print and fill out our patient forms and bring them to your first visit.

All patients under the age of 18 years of age must be accompanied by a parent or guardian at the appointment.


Patients taking drugs that affect blood clotting (such as Coumadin or Plavix) should notify their physicians prior to their procedure.


If you have current x-rays from your dentist or physician, you may request that they are forwarded to our office or you may bring the x-rays to your appointment. X-rays can be taken at our facility; however, this may result in additional costs to you.
