If you happen to have a complex medical history or treatment plan, however, you will require an evaluation and a second appointment, when our oral and maxillofacial surgeons will perform any necessary treatment.
We ask that you please provide the following information at the time of your appointment:
- Any X-rays from your doctor and your referral form, if applicable
- A list of current medications
- If you have coverage, your dental and medical insurance cards to bill the insurance company
- Medical records if you have a complex medical history
